Back in the Print Broker days, I did alot of work with Cedar Graphics. They were awesome in their attention to AA's. Job quoted as X. Bill came in as 2 or 3X because of legitimate AA'S. They were itemized and indisputable.
Boy did I learn to manage jobs a lot better, a lot faster, because I knew I was going to have a nasty conversation with my customer if I didn't.
When printer's eat the AA's they are actually doing no one a favor.
In tough times, some printers turn AAs into a sales method. I've heard more than one print sales manager say: "Low-ball the quote and we'll make it up on the alterations. And there’s always alterations."
